Fighters - CSAC Form Fighters will submit all  Completed documentation to CSAC forms including :

• CSAC fighter application license form

• Physical examination form completed/ signed by MD

• Pay CSAC fees 

• Please bring $10 for donation to the temple for hosting and facilities to pay homage and opening ceremony

 Cornerman Registration - All coaches/cornermen must be present :

• Fighters to submit cornermen list to CSAC

• CSAC application/ verification of Cornermen license

• Cornermen to receive wristbands during weigh-ins (wristbands are non-transferable)
